SCFHP is the County Health Authority for Santa Clara County, California. SCFHP was created by County ordinance and is a public agency. SCFHP's legal name is the Santa Clara County Health Authority, doing business as Santa Clara Family Health Plan. But SCFHP and the County of Santa Clara are separate legal entities. The County and its officials, employees and agents are not responsible for the obligations of SCFHP. SCFHP does not intend to, nor do they have the power to, confer on any person or entity any rights or remedies against the County or any officials, employees or agents of the County.
SCFHP is licensed by the California Department of Managed Health Care and is authorized to do business in Santa Clara County, California, only. Information that SCFHP publishes on this website may contain references or cross-references to products, programs or services of SCFHP that are not available in your county, state or country. The products referred to on this website may be offered and sold only to persons residing in Santa Clara County, California.

Texting Program Terms and Conditions
Text messaging campaigns are conducted with your prior express consent. We use a third-party vendor to help us manage our text messaging communications with you. At any time, and even if you have expressly given us permission to send text messages to you, you may communicate your desire not to receive additional text messages by following the stop or opt-out instructions in the text message or by contacting SCFHP’s Customer Service line.
- Your participation in SCFHP’s Texting Program, also referred to as the Wireless Text Message Services (the “Services”), is governed by the Terms and Conditions below. Use of the Services means that you have agreed to accept these terms, which are effective on the date you agree to participate. When you opt-in, you can expect to receive different healthcare notifications, such as, important reminders about your health care and coverage, and information about health and wellness. Message frequency varies.
- These Terms and Conditions are subject to change. SCFHP reserves the right to update or amend these Terms and Conditions at any time. In the event you do not agree with the updated Terms and Conditions, you may opt out of receiving text messages from SCFHP.
- SCFHP or its service provider may, at its sole discretion, terminate, adjust, or suspend the Services or any portion thereof, for any user for any reason. You can also terminate or suspend the Services at any time by texting the word “STOP” to 602055 to unsubscribe. For Help text HELP to 602055 or you can also call 1-800-260-2055 to reach SCFHP Medi-Cal Customer Service or 1-877-723-4795 to reach SCFHP DualConnect Customer Service.
- You must be at least 18 years old to be eligible to use the Services. SCFHP does not charge for the Services, however, message and data rates may apply. For example, your cellular phone service provider may charge you to receive wireless text messages. Neither SCFHP, nor its vendors, will be held responsible for any charges related to the use of the Services. You agree that you are responsible for paying your carrier’s charges to use the Services. SCFHP and Carriers are not liable for delayed or undelivered messages.
- SCFHP PROVIDES THE SERVICES "AS IS" WITH NO WARRANTIES OF ANY KIND. SCFHP EXPRESSLY DISCLAIMS ANY WARRANTY, EXPRESS OR IMPLIED, REGARDING THE SERVICES, INCLUDING ANY IMPLIED WARRANTY OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, OR THAT THE SERVICES WILL BE SECURE, UNINTERRUPTED OR FREE OF ERRORS, VIRUSES OR OTHER HARMFUL COMPONENTS.
- UNDER NO CIRCUMSTANCES WILL SCFHP OR ITS EMPLOYEES, OFFICERS, OR DIRECTORS BE LIABLE TO YOU FOR ANY DIRECT, INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL OR EXEMPLARY DAMAGES ARISING OUT OF OR IN CONNECTION WITH USE OF THE SERVICES WHETHER OR NOT SCFHP HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
- If you are dissatisfied with the Services or any content on any of SCFHP text messages, or with these Terms and Conditions, your sole and exclusive remedy is to discontinue using the Services. You acknowledge, by your use of the Services, that your use of the Services is at your sole risk.
- You agree to indemnify, defend and hold harmless SCFHP, its governing board, partners, clients, employees, consultants, agents, officers and directors, from and against any and all claims, liabilities, penalties, settlements, judgments, fees (including reasonable attorneys' fees) arising from (i) your use of the Services; (ii) your violation of these Terms and Conditions; and/or (iii) any violation or failure by you to comply with all laws and regulations in connection with the Services.
